ALIA PD record sheets [forms]

The summary of activities table members-only

  • helps you select your PD activities;
  • provides a description of the types of activities that are valid PD,
  • shows how points are allocated; and
  • documents the record keeping requirements for audit.

Count on being audited ... keep your PD file up to date

You need to maintain a personal record and reflection/learning outcome AS YOU GO of time spent on PD activities so it will not be an onerous task to get all your proof of compliance together to satisfy audit requirements.

You don't have to provide National Office with details of this record until you apply for a Certified Practitioner Certificate or are advised you have been selected for audit and required to establish proof of compliance - you will not be audited more than once in a 3 year period.

The 'ALIA record of learning activities sheet' is the documentary evidence used for the majority of PD activities.

Your personal online tracking database

You can choose to use the 'MyPD' recording mechanism to record and keep track of your PD activities or you can use the Annual Summary sheet with the Record sheets - either in hardcopy or electronically - for each PD activity category. Remember, you only need to provide detailed record sheets including your learning outcomes for the minimum 120 points per Triennium or 30 Points per PD Year.
