Excellence Award nomination form

The Excellence Award recognises excellence in any area, field or aspect of practice in the library and information services sector. Nominations should clearly identify the area, field or aspect of practice and demonstrate the basis on which the judgement of excellence has been made. Nominations should also demonstrate how the nominee's excellence in practice meets one or more of the Association's Objects.

At least two referee reports must accompany this application.

Use this form for applications for an Excellence Award. You should receive a confirmation of your nomination by email within two working days of submission.
